2005 ALA Annual
Stipend Contest Entry Form

LSO is offering a $500 stipend to attend the 2005 ALA Annual Conference in Chicago.

Unfortunately, ALA has already filled all the available slots for students to participate in their Student-to-Staff program. However, LSO is still offering students the opportunity to win a $500 stipend to attend the conference.

Eligibility
To participate, you must meet the following requirements:

Be a full time student during the Spring 2005 semester
Be a supporting member of LSO by the time of the drawing. (A supporting member is a student who has completed the LSO application form and paid the one-time membership fee of $20.)
Be willing to give a presentation of your conference experience to the LSO board

How to Enter
Complete the following form to be entered into our drawing. A student name will be drawn (raffle style) by SIRLS Director Jana Bradley on June 1, 2005.
